Olanzapine, as monotherapy or combination therapy, significantly improves symptoms in patients with manic or mixed episodes of bipolar affective disorder, according to data reported here at the 9th World Congress of Biological Psychiatry (WCBP) on June 29.
Pavel Vohlidka, MD, Eli Lilly and Company, Basingstoke, United Kingdom, presented 12-week outcomes in 251 patients who received olanzapine as acute-phase treatment either alone or in tandem with other antipsychotics, anticonvulsants and/or lithium, and antidepressants.
All patients were being treated for manic or mixed episodes of bipolar affective disorder and were enrolled in the 24-month Mania and Olanzapine Treatment (MANOLA) study, which examined patterns of clinical use of olanzapine in a natural setting over a recent 4-year period.
In the current analysis, about one-third of patients received olanzapine as monotherapy and about two-thirds received the agent as part of combination therapy. The groups were similar in terms of baseline demographic and clinical characteristics.
The primary objective of the analysis was to evaluate changes in mania symptoms in both groups on the Clinical Global Impression-Bipolar Disorder (CGI-BP) scale: overall, mania, depression, hallucinations, delusions.

Choice of Antipsychotics Influence Metabolic Abnormalities in Patients With Schizophrenia:Presented at APA
A prospective trial comparing 3 antipsychotic drugs revealed that clozapine is most likely to produce metabolic changes that point the way to metabolic syndrome.
However, clozapine was also most likely to control violent behaviour and metabolic consequences were most apparent for African American, researchers stated here at the 162nd Annual Meeting of the American Psychiatric Association (APA).
Menahem Krakowski, MD, New York University, New York, and Nathan Kline Institute, Orangeburg, New York, presented the findings of a randomised, double-blind study on May 20.
The study included 110 inpatients with schizophrenia or schizoaffective disorder who were randomised to receive clozapine (n = 34), olanzapine (n = 31), or haloperidol (n = 28).
At study entry, 93 patients gave blood samples for measuring fasting glucose, cholesterol and triglycerides and had at least 1 more sample collected during the 12 weeks of the study. The patients had a history of physical assaults.

FDA Approves Olanzapine, Fluoxetine HCl for Treatment-Resistant Depression
The US Food and Drug Administration (FDA) has approved olanzapine and fluoxetine HCl (Symbyax) capsules for the indication of acute treatment of treatment-resistant depression (TRD) in adult patients who have not responded to 2 separate trials of different antidepressants of adequate dose and duration in their current episode.
An 8-week randomised, double-blind, controlled study evaluated the efficacy of olanzapine and fluoxetine HCl in patients (n=300) who met the fourth edition of Diagnostic and Statistical Manual of Mental Disorders (DSM-IV) criteria for major depressive disorder (MDD) and did not respond to 2 antidepressants of adequate dose and duration in their current episode.

Olanzapine Long-Acting Injection (LAI) Data Presented at First Annual Schizophrenia International Research Society Conference
Results from olanzapine )long-acting injection (LAI) clinical trials showed that the efficacy and safety profile of olanzapine LAI was generally consistent with that of Zyprexa(R) (olanzapine) with the exception of injection-related events. Results from a 24-week maintenance study (HGKA) and interim findings from an ongoing open-label study (HGKB) were presented at the first annual Schizophrenia International Research Society (SIRS) Conference in Venice, Italy.

New Findings in Early-Onset Schizophrenia
The Treatment of Early-Onset Schizophrenia Spectrum Disorders (TEOSS) study, funded by the NIMH, was a multicenter, controlled treatment trial of children and adolescents.1 The aim of the TEOSS study was to compare the efficacy and safety of risperidone and olanzapine with that of a traditional antipsychotic agent—molindone.
In this double-blind, parallel-group study, patients were randomized to receive risperidone, olanzapine, or molindone for 8 weeks followed by a 44-week, double-blind maintenance phase for responders. A total of 119 children and adolescents were randomized in the study. The results of the study, which should be reported in the near future, will provide needed information about the safety and tolerability of antipsychotic medications in youths with schizophrenia spectrum disorders.
The clinical characteristics of this large sample were recently reported and provide important information about the demographics and severity of illness in children and adolescents with early-onset schizophrenia and schizoaffective disorder.2 The mean age of onset of illness was 11.1 years. At the time of enrollment, the mean age of the patients was 13.8 years. The sex distribution was 66% male and 34% female. The racial breakdown was 62% white, 31% African Amer-ican, 3.4% Asian, 2.5% mixed, and 0.8% Pacific Islander. The average IQ of the sample was 93. Most of the youths lived with their family, 4% were hospitalized, and 7% lived in a group home or residential facility.

Clozapine Better Than "High-Dose" Olanzapine for the Treatment of Adolescents With Schizophrenia

Comparison of antipsychotic treatments in adolescents with schizophrenia
There is a wealth of scientific literature available on the treatment of adults diagnosed with schizophrenia. However, there is a paucity of data to guide the treatment of children and adolescents with schizophrenia. “Although the U.S. Food and Drug Administration (FDA) has recently approved the use of aripiprazole and risperidone for adolescents with schizophrenia, few controlled data are available to help guide clinicians regarding the management of children and adolescents with schizophrenia who fail to respond to these standard 'first-line' antipsychotic treatments,” according to Dr. Sanjiv Kumra. Dr. Kumra is one of the authors of a new study to be published in the March 1st issue of Biological Psychiatry, which was undertaken to help fill this gap in knowledge.
The authors recruited 39 children, 10-18 years of age, who had already failed to respond to at least two antipsychotic treatments, to participate in a 12-week, double-blind, randomized study – the most rigorous of clinical trial designs. After initial assessments, the patients received treatment with either clozapine or “high-dose” olanzapine (doses that exceed the package insert recommendations) and were monitored for improvement in their symptoms. The researchers discovered that clozapine was approximately twice as likely to produce a treatment response as olanzapine. Both positive symptoms (psychosis) and negative symptoms (blunted emotional response, reduced motivation) responded better to clozapine. John H. Krystal, M.D., Editor of Biological Psychiatry and affiliated with both Yale University School of Medicine and the VA Connecticut Healthcare System, comments on the findings: “Olanzapine is among the most effective antipsychotic medications, so the distinctive effectiveness of clozapine in this study could be very important.”
